Jiri Fischer joined the Czech team for WC. So, if no other players refuse to play, the final roster might look like that.

Zidlicky - Spacek
Slegr - one of the rest (Fischer, Malik, Kaberle F.)
The first 3 D-men are pretty much a sure thing IMO, Slegr will play because he is Hlinkas favourite, Spacek has got a lot of international experience.
Havlat - Straka - Jagr
Straka can play both center or a left wing, he will sure play with Jagr, Havlat probably joins them cause together they showed some super chemistry on the Olympics.
Rucinsky - Reichel - Dvorak
Clear choise, have played a lot int. games together.
Elias - Cajanek - Sykora
Elias - Sykora looks to me clear as well but am not sure who will center them, might be Dopita as well.
Prospal - Vasicek - Vlasak
Well, the rest. Prospal might also play the center role
